Please make sure we know about any special health needs or conditions your child may have.

All consultations are strictly confidential.
HEEADSS assessments:
Adolescence is a time of growth and development and can be challenging for some.
The Student Health Nurse will be conducting health checks on students using the Ministry of Health’s “HEEADSSS” assessment tool. This includes a range of questions relating to home, school, eating, drug/alcohol, depression/mental health, sexuality and safety.
Any health risks identified will have the appropriate support offered to students and families.
NOTE: You have the option of opting out of this when filling out the Health Form found in the school enrolment packs.
